The information collected from this survey will inform CNET’s ongoing initiative to help standardize NET patient care in Canada. Our goal is to improve efficiency, reduce costs, and lead to better outcomes by bringing advanced cancer care closer to home.

A method that may be helpful to our initiative to standardize NET care is a shared care model (SCM) where doctors from advanced care centers and the community setting work together to discuss best practices for diagnosing and treating NET patients.
The survey contains 43 questions and will take approximately 15 to 20 minutes.
This survey will ask you general questions about where you call home and where you receive your cancer care, some specifics about your cancer (namely the stage, progression, and management), and your attitudes towards and your experiences with SCM as it relates to your care. The questions may ask you to select one choice from a list, select multiple answers, rank your feelings on a scale from strongly disagree to strongly agree, or write some brief comments.
This survey may be completed independently or with the help of a trusted family member, friend, or caregiver at any stage during its course.
This survey is only available in English.
Participation in this survey is voluntary. You may exit the survey at any time and all your answers will be deleted. At the end of the survey, there will be the option to either submit or void the answers you have given. Answers will only be collected from completed surveys whereby the results have been chosen to be submitted by the participant. There are no restrictions to other studies you may be involved with when agreeing to participate. Neither your willingness to participate nor your answers will impact your quality of care.
By completing the survey and choosing to submit your answers you are consenting to the use of this information in our study.
This survey is the only component of this study you will be asked to be involved with, and the only other time you will be contacted regarding this study is when the results are published so you may choose to view them.
